Patient Financing

Thank you for choosing Larson Eye Care for your eye care needs. Larson Eye Care has established a financial policy to promote positive communication between our office and you, the patient we serve. We are happy to assist you with questions you may have regarding your account and to work with you regarding the processing of your insurance claim. The following information outlines our payment policy.

As the patient, you are financially responsible for all services provided by Larson Eye Care.

Your Medical Insurance

Your medical insurance policy is a contract between you and the insurance carrier. Please take time to read and understand the procedures outlined in your policy handbook provided by your insurance company. We will file all claims for our services with your primary and secondary (when applicable) insurance companies.

Co-payments, Deductibles and Coinsurance

Co-payments, Deductibles, and Coinsurance - We contract with many area employers, HMO’s, PPO’s, various insurance companies, Medicare and Medicaid. Because of our contractual obligation with these plans, we have agreed to accept a specific reimbursement for our services. Once your insurance has provided payment, you will be billed for any remaining balance that may be due.

Please pay your Co-Payment at the time of your office visit

We cannot waive any co-payments, deductibles or coinsurance amounts defined as patient responsibility. Such a waiver may violate state and federal laws. If you are unable to pay the co-pay at the time of service, we would be happy to reschedule the appointment. If you prefer to keep the appointment and be billed for the co-pay, a $10 statement fee may apply.

Prior Authorization and Referral Forms

If your insurance plan requires you to have a referral form prior to an office visit, please obtain the proper referral prior to your appointment. If a referral is not obtained, your appointment may have to be rescheduled so that you are not financially responsible. We will pre-authorize any scheduled procedure on your behalf. If your insurance coverage changes prior to the scheduled procedure, it is your responsibility to notify Larson Eye Care.

Insurance Patients

After your insurance has reimbursed Larson Eye Care, you will receive a statement if there is a remaining balance due.
Payment in full is due upon receipt of the statement unless payment arrangements have been made with the billing office. We accept payment by cash, check, Visa, MasterCard and Discover. There will be a $25.00 fee for any returned checks. Parents are responsible for all fees and services rendered to a minor.

Self Pay – No Insurance on File

If you have no insurance, choose not to use your benefits, or you request a service that is not covered by your insurance, you will be asked to pay for your services at the time of your visit and in doing so may qualify for a prompt pay discount. If you pursue payment from insurance independently, Larson Eye Care may discontinue any self-pay related discounts and pursue the full amount of charges. We accept payment by cash, check, Visa, MasterCard and Discover. There will be a $25.00 fee for any returned checks. Parents are responsible for all fees and services rendered to a minor.

Missed Appointments

Missed Appointments - We certainly understand if you need to cancel your appointment due to illness or emergency. However, after a second occurrence of canceling an appointment within 24 hours of the appointment or not showing for an appointment, this may result in a $25 fee payable by you per occurrence.

Minors

Parents are responsible for all fees and services rendered to a minor unless minor is deemed to be emancipated or have similar status. Larson Eye Care will not waive responsibility of parents in circumstances of custodial dispute.

Methods of Payment

We accept cash, check, MasterCard, Visa and Discover. Financing is available through Care Credit. There will be a $25
fee for any returned checks.

Collection Agency Placement Policy

You are financially responsible for the timely payment of your outstanding bill per our Financial Policy. You will be responsible for any and all collection agency fees up to 33% for the amount placed with the collection agency. In the event we need to seek legal action for collection on your account, you will also be responsible for any and all fees associated with court costs, garnishments and/or attorney fees.
