Insurance Coverage

Medical Insurance

Larson Eye Care is a participating Medicare provider and contracts with many area employers, HMO’s, PPO’s, various insurance companies, Medicare, and Medicaid to better serve our patients. Because of our contractual obligation with these plans, we have agreed to accept a specific reimbursement for our services. Your medical insurance policy is a contract between you and the insurance carrier. Patients are responsible for reading, understanding and following the procedures outlined in the policy handbook provided by their insurance provider.

Co-payments, Deductibles, and Coinsurance

Co-payments, deductibles and coinsurance payments are the responsibility of the patient and are due at time of service. We cannot waive any co-payments, deductibles or coinsurance amounts as such waivers may violate state and federal laws. For your convenience, we accept Visa, MasterCard and Discover. If you are unable to pay the co-pay at the time of service, the appointment will need to be rescheduled or a $10 statement fee may apply. Our insurance department is always available to help answer any questions you may have regarding your insurance coverage.

Prior Authorization and Referral Forms

If your medical insurance plan requires you to have a referral form or pre-certification prior to a procedure or a visit to our office, please obtain the proper referral before arriving for your visit. Referral forms may be faxed to our office in advance of your appointment to 920.452.1920.

Insurance Payments

Once your insurance provider has reimbursed Larson Eye Care for your care, patients will receive a statement of any outstanding balances. The patient responsibility is payable upon receipt. We accept payment by cash, check, Visa, MasterCard and Discover.

Visit Our Cataract Center

Cataract patients now have the option to have their vision corrected to allow them to see at both near and far distances after cataract surgery! These lens implants provide significantly reduced dependence on glasses for all tasks of daily living, providing a range of focus that laser vision correction cannot achieve. Individuals with visual impairment suffering from cataracts previously had only a mono-focal lens implant option after surgery.
